Welcome to our new Care Coordinator, Jordan Palker, MSW

We are very pleased to welcome Jordan Palker, MSW to our Lakeside Pediatrics team as our new Lead Care Coordinator!

Jordan brings extensive experience in family support and care management and will work closely with our nursing and administrative staff.  We are proud of our comprehensive care for children with complex medical and behavioral health needs, and for all of our children and families.  Working with the University of Vermont Community Health Team and OneCare Vermont, Ms. Palker will assist us in our redesign and expansion of our care coordination services.
